This is a directory, not a recommendation — a list of licensed long-term care homes in Ontario, organized by municipality, drawn from the Ministry of Health’s public service-provider registry. It does not rank homes or comment on care quality.

The list matters most once a family is already making decisions. If a parent or spouse is moving into long-term care, it is often the moment powers of attorney for property and personal care actually come into use, and the moment capacity gets tested in a practical way — signing admission paperwork, managing finances from outside the home, or deciding who can act if disagreement follows. Knowing which home a family member is in, and its municipality, also matters later for probate and estate administration, since it affects where records and assets are held.
