Key takeaways
  • Choosing the right form depends on your problem, such as unpaid rent, a tenant moving out, or a maintenance issue.
  • Landlord fees for forms such as L1 and L2 are $201 ($186 through the Portal), while tenant forms such as T2 and T6 are $53 ($48 through the Portal).
  • You can file through the Portal, by email, by mail or courier, or in person at ServiceOntario.
  • An individual may be able to request a fee waiver.

The Landlord and Tenant Board uses many different application forms, each with a letter and number. Filing the wrong one can cost you time and money, and LTB fees are non-refundable.

This guide matches common landlord and tenant problems to the LTB form that usually fits, and explains fees and filing. It is general information and not legal advice.

Start with your problem, not the form number

Begin by describing in one sentence what you want: to end a tenancy, to collect money, to get repairs, or to find out whether the law applies at all. Then match that goal to a form. The LTB resolves disputes through mediation or through a hearing and order.

Common landlord forms

FormWhat it is for
L1Evict for non-payment and collect rent; needs an N4 notice first
L2End the tenancy and evict
L3Tenant gave notice or agreed to end the tenancy
L4Tenant breached a settlement or order
L9Collect rent from a tenant still in the unit
L10Collect money from a former tenant

For L10, the tenant must have moved out on or after September 1, 2021, and the application must be filed within one year of move-out.

Common tenant forms

FormWhat it is for
T1Rebate of money the landlord owes
T2Tenant rights and interference
T5Bad faith notice
T6Maintenance problems
A1Whether the Act applies to your arrangement

Some problems do not belong at the LTB. A tenant cannot file an LTB application against a co-tenant or roommate, and discrimination in refusing to rent goes to the Human Rights Tribunal of Ontario, except for deposit and handover issues under RTA section 107.

An individual, but not a business or organization, can request a fee waiver by completing all three parts of the LTB Fee Waiver Request form, including the Declaration, and attaching it to the application. If the waiver is denied and payment information was included, the LTB processes payment immediately. Otherwise you must pay within three business days of being contacted or the application is returned.

How to file and pay

The Tribunals Ontario Portal accepts forms including L1, L2, L3, L4, L9, L10, T1, T2, T5 and T6. Others, such as L6, L7, L8, A1, A2 and A4, are filed by email with online payment. You can also file by mail or courier to 15 Grosvenor Street, Ground Floor, Toronto, ON M7A 2G6, or in person at about 80 ServiceOntario locations.

Documents cannot be mailed or faxed to ServiceOntario locations, and ServiceOntario staff cannot give legal or procedural information.

For email-filed applications paid online, pay first, then email the application and receipt to ltbpayments@ontario.ca within 24 hours with the 7-digit receipt number in the subject line, otherwise the LTB will refund the payment. Credit card details cannot be sent by email or fax.
